Abstract

Pelaksanaan Provider Initiated Testing and Counseling (PITC) merupakan strategi penting dalam deteksi dini HIV di fasilitas pelayanan kesehatan. Namun, evaluasi program HIV selama ini masih cenderung berorientasi pada capaian administratif dan belum banyak menggunakan positivity rate sebagai indikator mutu pelayanan. Penelitian ini bertujuan menganalisis positivity rate PITC sebagai indikator mutu tersembunyi pelayanan HIV dalam perspektif keselamatan pasien di RS Bhayangkara TK II Medan tahun 2025. Penelitian menggunakan pendekatan kuantitatif dengan desain deskriptif analitik retrospektif menggunakan data sekunder laporan bulanan PITC dan PMTCT periode Januari–Desember 2025. Variabel penelitian meliputi jumlah testing HIV dan jumlah hasil HIV positif. Analisis dilakukan secara deskriptif melalui perhitungan positivity rate bulanan dan interpretasi epidemiologis terhadap pola skrining HIV rumah sakit. Hasil penelitian menunjukkan bahwa jumlah testing HIV masih rendah dan fluktuatif, namun positivity rate pada beberapa bulan tergolong sangat tinggi, yaitu Januari 16,7%, Maret 50%, April 60%, Juli 50%, dan November 28,6%. Temuan ini menunjukkan bahwa testing HIV kemungkinan masih dilakukan secara selektif pada pasien dengan probabilitas klinis tinggi sehingga berpotensi menimbulkan keterlambatan diagnosis HIV dan missed opportunity testing. Penelitian ini menyimpulkan bahwa positivity rate dapat digunakan sebagai indikator mutu tersembunyi pelayanan HIV serta mencerminkan efektivitas deteksi dini HIV dalam sistem pelayanan rumah sakit.
